The social workers at Matulaitis provide a caring and supportive relationship with residents and their families even before they enter our building. On admission, the Social Services Department assists the resident and their families with ongoing supportive counseling to ensure that their social-emotional needs are met. Individualized care plans are developed that include adjustment to the facility, adjustment to physical changes and the emotional issues that are impacted by care needs. Ensuring that our residents are comfortable and secure during their stay and that they maintain the highest standard of care is of utmost importance to our staff. The social workers also provide support to the families of residents during the resident’s stay at our facility.
The goal for short-term admissions is to receive rehabilitation and nursing services that will enable residents to regain their highest level of functioning and to get back to active living. The Social Services Department is part of an interdisciplinary team that prepares a resident to return to the community setting. The Social Services Department often makes referrals for home health care services and assists with ordering adaptive equipment that ensures optimum independence once a resident has been discharged.
The Social Service Department acts as a liaison between the resident and his or her insurance carrier. Further, our social workers assist residents and families with preparing Medicaid applications to the State of Connecticut Department of Social Services as needed. Residents are assisted with personal money accounts upon request.
